Sprains, ligaments and why some ankles keep rolling

A sprain happens when the ligaments on the outside of your ankle get stretched or torn, usually when the foot rolls inwards. When they are overloaded past what they can handle, they tear to some degree, and that is what causes the swelling, bruising and pain. Physios usually describe ligament injuries in three grades, and it helps to know roughly where yours sits.

The grade guides the plan but does not decide everything. What matters just as much is how well the ankle recovers its strength and balance afterwards. An ankle keeps rolling when the muscles and the sense of position around the joint were not retrained after the first injury, so it no longer reacts quickly enough to catch itself. The good news is this is very treatable. Targeted balance and strength work rebuilds that control and breaks the cycle.

Grade 1

The ligament is overstretched with only minor tearing. Some swelling and tenderness, but the ankle still feels fairly stable.

Grade 2

A partial tear. More swelling and bruising, more pain on weight bearing, and the ankle can feel loose.

Grade 3

A full tear. Significant swelling, difficulty walking and a clear sense the ankle is unstable.
