Most vertigo comes from the vestibular system, the balance organ in your inner ear that tells your brain where your head is in space. When that system sends the wrong signals, you feel the world move when it is not.
The most common cause we see is BPPV, benign paroxysmal positional vertigo. It happens when tiny crystals in the inner ear come loose and move into a part of the ear where they do not belong. When you move your head, those crystals shift and trigger short, intense bursts of spinning. It sounds alarming, but BPPV is mechanical and well understood, which is exactly why it responds so well to treatment.
Vertigo and dizziness can also come from other vestibular causes, such as inflammation of the inner ear nerve or a balance system that has not fully recovered after illness. Part of what we do at your first visit is sort out which is at play, because the right treatment depends on the right diagnosis. If your dizziness comes with other symptoms that point away from the inner ear, we will tell you and help you get to the right person.
Treatment starts with a proper assessment. We take you through some simple position changes and watch how your eyes and your symptoms respond, which tells us which ear is involved and what type of vertigo you have. For BPPV the most effective treatment is canalith repositioning, most often the Epley manoeuvre, a precise guided sequence of head and body movements that walks the loose crystals back to where they belong. It is done in the clinic, involves no medication, and many people feel a clear difference straight after. Where your balance system needs to relearn and adapt, we prescribe vestibular rehabilitation exercises that retrain your brain to handle the signals from your inner ear.
